Remote Health Monitoring

With Remote Health Monitoring, advanced medical devices are shipped to your home so we can monitor your vitals and blood sugar measurement in real time. Based on the readings, the appropriate changes to your care can be made promptly.

Cornerstone RPM utilizes an ATT cellular connection instead of Bluetooth to ensure your data is secure and transmitted efficiently.

Your nurse navigator monitors your vitals daily and can promptly intervene with any abnormal reading.

We will follow up with your doctor if changes need to be made to your medications or care plan.

Remote Nurse Navigator (RNN)

With Cornerstone RPM, you are paired with a dedicated nurse who supports you throughout your healthcare journey, ensuring your safety and advocating for your optimal health. 

Each customer is assigned a Nurse Navigator who provides:

Regular wellness checks (Monthly or weekly based on medical necessity)

Follow-up on vaccinations, screenings, and specialist visits

Personalized Telehealth Service

Your nurse or one of our in-house providers can join a voice or video call with you to assess acute medical conditions such as skin problems, wounds, respiratory illnesses, allergies, and more. We do not provide prescription or diagnostic services; however, our team is able to guide you in the right direction.
